Submit a Leave of Absence Request


On the Request Leave of Absence page, employees can provide all information relevant to their leave case including the Reason for the leave and the dates of the leave. Employees may also be able to define the leave type, such as whether the requested leave is continuous or intermittent. Each Leave Type serves different needs and results in different corresponding fields. 

  1. Continuous Leave: Continuous leave is time taken in a single continuous block. Examples of continuous leave may include leave following the birth or adoption of a child or recovery from a major surgery. 
  2. Intermittent Leave: Intermittent leave is time taken in intervals instead of all at once. An example of intermittent leave would be weekly physical therapy appointments that take place every Wednesday for three months. 
  3. Intermittent Weekdays Leave: This option is for intermittent leave that is cyclical for a set number of weeks. For example, an employee may take personal leave for a class that runs on a two-week cycle where one week's schedule is Monday, Wednesday, Friday, and the second week is Tuesday and Thursday. 


Submit a Leave of Absence Request 

Navigation: Menu > My Info > My Time > Leave > Request 

  1. From the Leave of Absence Request page, select the applicable reason for the leave at the Reason field. 
  2. Enter the Start Date and Estimate Return Date at the respective fields. 
  3. In the Request Type section, select the appropriate type.
  4. Complete the fields that result from the Request Type selection. 
  5. (Optional) Enter comments in the Comments field. 
  6. Select Submit. 

Note: Some companies may have a qualifier questionnaire configured to help employees determine if they are eligible for leave. This questionnaire is initiated from the More actions (…) menu and then completed from the employee’s To Do items.
