What is a 1095-C Form? 

Form 1095-C is filed and provided to any employee of an Applicable Large Employers (ALE) member who is a full-time employee for one or more months of the calendar. ALE members must report that information for all twelve months of the calendar year for each employee. 


The IRS (Internal Revenue Service) uses the information provided on this form to determine whether you or your employer may have to pay a fine for failing to comply with the Affordable Care Act, or ACA (Affordable Care Act), by not having healthcare coverage for that year.  


If an employee is eligible for healthcare coverage from their employer, Form 1095-C contains information about this coverage offered to them and their dependents from their employer. 1095-C is used by qualifying employers with 50 or more full-time employees (including full-time equivalents) that are subject to the employer responsibility provisions of the ACA. Information on this form is required for employers to prepare and file their annual tax return.
